1Who we are

ELF Insurance, LLC ("ELF," "we," "us") is an independent insurance agency based in Mesa, Arizona, licensed in Arizona for life and accident and health insurance (agency license #3001321556, NPN 19920789). This policy explains how we handle personal information we collect through our website, social media pages, online forms, scheduling tools, phone calls, texts and email.

As an agency, we work with several insurance companies. When you apply for a policy, the insurance company also collects and uses your information under its own privacy notice, which it will give you.

2Information we collect

Information you give us

• Contact details: name, phone number, email address, ZIP code and the best time to reach you.
• What you're interested in and anything you tell us in a message, form, call or appointment.
• When you request a quote or apply: date of birth, gender, height and weight, tobacco use, health history, beneficiaries, income and other details the insurance company requires.
• Payment details you give directly to an insurance company for premiums. ELF does not store card or bank account numbers.
• If you're an agent interested in joining ELF: license status, experience, the products you sell and similar career information.

Information from other sources

• Lead forms and messages you submit through Facebook, Instagram, Google or our booking page.
• Insurance companies, about the status of your application or policy.
• Public licensing databases, when we verify an agent's license.

3How we use it

• To respond to your request, schedule your review and follow up as you've asked.
• To compare coverage, run quotes and illustrations, and help you apply.
• To service your policy after the sale, including reviews and beneficiary or address changes.
• To send information about insurance products you may be interested in, when you have agreed to receive it.
• To meet our legal, licensing and carrier requirements, keep records and prevent fraud.
• To review and improve our website, posts and advertising.

4Calls, texts and email

When you check the consent box on one of our forms, book an appointment, or otherwise give permission, you agree that ELF Insurance, LLC may contact you about insurance by phone, text message and email at the number and address you provided, which may include automated technology. Consent is not a condition of purchase. Message and data rates may apply, and message frequency varies.

• Reply STOP to any text to stop texts, or HELP for help.
• Ask us at any time to stop calling or emailing you, and we will add you to our internal do-not-contact list.
• We check the National Do Not Call Registry before calling anyone who has not given us permission.
• We do not share or sell your phone number or text opt-in consent to third parties for their own marketing.

5Who we share it with

We do not sell your personal information. We share it only as follows:

• Insurance companies you ask us to quote or apply with, so they can underwrite and issue your policy.
• ELF agents who are licensed to help you, and our marketing organization, which supports our contracts with insurance companies.
• Service providers who help us run the agency, such as email, scheduling, document storage, lead tracking and advertising platforms. They may use your information only to provide services to us.
• Government and legal when required by law, regulation, subpoena or a request from an insurance regulator, or to protect against fraud.
• A successor business if ELF's book of business is sold or transferred, subject to this policy.

6Health and insurance information

Health and financial details you share to get a quote or apply are used only to help you get coverage. We share them only with the insurance companies you choose and as the law allows.

Arizona's Insurance Information and Privacy Protection Act (A.R.S. § 20-2101 and following) and the federal Gramm-Leach-Bliley Act give you rights over personal information collected in connection with insurance, including the right to request access to it and to ask for corrections. Insurance companies may also check consumer reporting sources, such as the MIB, during underwriting; they will tell you when they do.

7Websites, social media and cookies

Our website uses the Meta Pixel, a small piece of code from Meta (the company behind Facebook and Instagram). It uses cookies to record visits and actions on our site, such as viewing a page or sending a contact form, so we can measure our ads and show relevant ads to people who have visited. Meta may combine this with other information it has about you under its own privacy policy. We do not send health information or the content of forms to Meta. You can limit this through your browser settings, your Facebook and Instagram ad preferences, or by opting out at facebook.com/adpreferences.

When you contact us through a social media platform, that platform's own privacy policy also applies to what you share there. Please don't post health or financial details in public comments; send them in a private message or bring them to your review.

8Security and retention

We use reasonable safeguards to protect your information, including password-protected accounts, two-step verification where available, and limiting access to people who need it. No system is perfectly secure, so we can't guarantee absolute security.

We keep records for as long as needed to serve you and to meet insurance record-keeping laws and carrier requirements, then delete or destroy them securely.

9Your choices and rights

• Opt out of calls, texts or email at any time, as described in section 4.
• Ask what personal information we have about you, and ask us to correct it.
• Ask us to delete information we don't need to keep for legal or carrier reasons.

To make a request, contact us using the details below. We may need to confirm your identity first, and we'll respond within 30 business days.

10Children

Our website and social pages are meant for adults. We don't knowingly collect personal information from anyone under 18 except from a parent or guardian applying for coverage on a child's behalf.

11Changes to this policy

We may update this policy as our services or the law change. We'll post the new version here with a new effective date.
